Information for Summer Program Providers

It’s almost the Summer Camp Season! We are delighted to be working with you.

We have a number of administrative requirements that you will need to complete before the start of our summer programs.

Please check our 2020 Summer Camp Brochure to review the camps that you are offering this summer.  As well, please  look for these camps on our  website.  Once you login you can check your registration numbers as well.
 
  1. We will also need your W-9 form (if new) with Birmingham Public Schools. Click here for the W-9 form.
 
  1. Please forward a valid certificate of liability insurance that includes a $1 million dollar per occurrence and a $2 million dollar general aggregate and names Birmingham Public Schools as the additional insured or indemnified party. Please use the address of 31301 Evergreen Road, Beverly Hills, 48025 for Birmingham Public Schools but email the certificate directly to Diane Rampolo at drampolo@birmingham.k12.mi.us. Insurance certificates must be received 2 weeks prior to the start of your camps or your camps will be cancelled.
 
  1. Fingerprint results are required for you and your staff. Please keep in mind that these document requirements are for your protection and the safety of our campers. Click here for the appropriate forms (Live Scan Request, Locations, Finger Print Information and Finger Print FAQ) for the initial fingerprints. NOTE: Fingerprint results for you and your staff are only valid IF you and your staff have worked continually (i.e. no break in employment over 30 days) for Birmingham Public Schools. Fingerprints can be transferred from another school district. You can make that request at the institution who received the initial fingerprint results. Those results should be sent directly to Dawn Nofzinger in the Human Resource department at DNofzinger@birmingham.k12.mi.us.
 
  1. Contractors/instructors are required to submit consent forms for I-Chats on all staff members by June 1, 2020. Click here for the form.  Each staff person on site is required to complete an I-Chat form.
 
  1. All camp contractors and their employees must participate in the safety training below. Click here for the information how to log in to complete the Safety Training.
  • Concussion Awareness: Mandatory for all contractors and their staff – Complete Course – 20 minutes.
  • Health Emergencies: Life-Threatening Allergies – Mandatory for all contractors and their staff – Complete Course – 13 minutes.
 
  1. An adult must be present at all times during your camp, a roster of your staff member(s) with cell phone number(s) must be made available to our office on the first day of camp and you must be able to document who was present in the BPS facilities at the time of your camp.
